Bloom360, Inc. This nondiscrimination policy applies to all of our programs, services, and activities, including but not limited to enrollment, access to care, and treatment by our healthcare providers and staff.
Bloom360 does not exclude people or treat them differently because of race, color, national origin, age, disability, or sex. We provide equal access to our telehealth services for all eligible individuals.
Accommodations and Auxiliary Services
Bloom360 is committed to providing reasonable modifications and auxiliary aids and services to individuals with disabilities to ensure effective communication and equal access to our Services. Available accommodations include but are not limited to:
- Qualified sign language interpreters (video relay services)
- Written materials in accessible formats (large print, audio, accessible electronic formats)
- Assistive technology compatibility for our telehealth platform
- Reasonable modifications to policies, practices, and procedures
Bloom360 also provides free language assistance services to individuals whose primary language is not English, including qualified interpreters and information written in other languages.

How to File a Grievance
If you believe that Bloom360 has failed to provide these services or has discriminated in any way on the basis of race, color, national origin, age, disability, sex, or any other protected characteristic, you may file a grievance with our Civil Rights Coordinator:

You may file a grievance in person, by mail, or by email. We will acknowledge receipt of your grievance within five (5) business days and provide a written response within thirty (30) days. If you need assistance filing a grievance, our Civil Rights Coordinator is available to help you.
